This is a two day to one week externship supervised by Dr. Brent Lin, Clinical Professor of Pediatric Dentistry and Director of the Pre-Doctoral Pediatric Dentistry Program. Externs will only observe and will not be involved with patient services. If you are interested in applying, please review the requirements below and e-mail the completed application documents in one pdf document to Judy Huang (judy.huang (at) ucsf.edu). The review of your application will be delayed if any documents and/or application fee are missing. Please allow up to two weeks for Departmental review of your completed application. If your application is accepted by the Department, it will be forwarded to the UCSF School of Dentistry Dean’s Office for review and final approval.
Requirements:
- Availability of two days to one week. Please provide the dates that you would like to schedule your visit. The dates must be at least three to four months from the date that your application is received by Judy Huang.

- Dean’s Letter confirming academic good standing or a letter from employer or colleague stating good standing in clinical practice if you are currently in practice
- Proof of malpractice insurance

- Badge Request Form (please complete the 1st section only)
- TB Screening Forms
- A government issued photo ID (passport, driver’s license, etc.)
- $95 ($500 for international) application processing fee made payable to UC Regents. This is a non-refundable fee. If you are not able to commit to your visit dates after your application is approved by the Department, you may reschedule once for an additional fee of $60 and you must resubmit the paperwork to reflect the new visit dates.
